General Information

At Mosaic Relationship Center (MRC), the approach to the cost of services is designed to be transparent and straightforward, prioritizing accessibility while maintaining high-quality care. MRC operates on an out-of-pocket payment model, which means that the center does not accept insurance. This decision is grounded in a desire to provide clients with unhindered, personalized sessions, free from the constraints and privacy concerns that can accompany insurance-based billing. The following billing rates are subject to change:

  • Intake Session (1st Session): $185
  • Individual Session: $130
  • Couples Session: $150
Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.

Superbill Submittals

A superbill is different from a simple receipt or ordinary invoice. It serves as the primary data source that a payer (insurance, funds, or programs) will use to construct a healthcare claim to calculate reimbursement against a healthcare service. Insurance companies demand that a superbill have specified information. Patients will need to check with your insurance provider when it comes to superbill submittals.

Essentially, a Superbill is an itemized list of all services provided to a client. The Superbill will also contain additional information about the patient visit including practice information, CPT codes, ICD-10 codes, referring doctors and more.
